Guidelines for submitting your application.

Your application should be in the form of a single PDF document with the following sections:

·   A brief, one-or-two-page, description of the project to be funded including date(s) and cost.
·   A list of key administrative personnel and artist(s) including addresses, phone, and email. Brief artist resumes may be attached.
·   Your annual budget. (You may use your form or any standard form for this including the Miami-Dade County Department of Cultural Affairs form.)
·   Your 501-c-3 determination letter from the Internal Revenue Service.
·   Your current Florida Department of Agriculture and Consumer Services registration under The Solicitation of Contributions Act.
·   Your most recent IRS Form 990 or 990-EZ filing including proof of acceptance.
